- DS-11 Form Filled out
- Proof of US Citzenship
- Government Issued ID
- Color Passport Photo
- Fee Payment (money order, check for US Department of State and City of El Paso)
Additional Requirements for Children
- Proof of relationship to child
- Both parents present or consent form signed by absent parent
